California Department of Aging (CDA) Agreement Number MS-0809-13

 

RECOMMENDATION:

Adopt a Resolution:

1.

Authorizing the President of the Board to execute Agreement Number MS-0809-13 with the California Department of Aging, for funding in the amount of $1,542,600 for the continuation of the Multipurpose Senior Services Program for the term of July 1, 2008 through June 30, 2010; and

2.

Authorizing the Director of Health or the Director's designee to execute contract amendments which modify the funding amount by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.

 

VISION ALIGNMENT:

Commitment: Ensure basic health and safety for all.

Goal 8: Help vulnerable people–the aged, disabled, mentally ill, at-risk youth and others–achieve a better quality of life.

 

Multipurpose Senior Services Program (MSSP) funding contributes to this commitment and goal by allowing Aging and Adult Services (AAS) to provide clients with adult day care, attendant care, home health coordination, emergency response systems, home modifications, adaptive equipment, and transportation services, as well as comprehensive case management services.

 

The following measure shows how the services provided through MSSP, assist in avoiding placement in a skilled nursing facility.

BACKGROUND:

It is anticipated that a total of 250 clients will be served through MSSP funding annually. It is projected that 22 clients (nine percent) may need to be placed in a skilled nursing facility due to their physical deterioration. MSSP, one of the AAS’s case management programs, provides comprehensive social and health care management services to Medi-Cal eligible at-risk elders.

 

The primary objectives of MSSP are to avoid the inappropriate placement of fragile older persons in nursing facilities and to foster independent living in their own communities. MSSP provides a care management team of social workers and public health nurses to assess client eligibility and need and to arrange and monitor the delivery of services. A client becomes ineligible for MSSP when the program’s services are no longer sufficient to enable the individual to remain safely at home. Though many clients stop participation because of necessary institutionalization, many others are able to avoid institutionalization altogether through MSSP’s services and are able to remain at home as clients with the program for the remainder of their lives.

 

DISCUSSION:

The Agreement includes funds for the MSSP social workers and nurses to provide assessments and case management services. Funds are also made available through the Agreement to provide home modifications, to purchase durable medical equipment and other supplies, and to provide attendant care.

 

County Counsel has reviewed and approved the Agreement and Resolution as to form.

 

FISCAL IMPACT:

The term of this Agreement is July 1, 2008 through June 30, 2010. Execution of this Agreement with CDA allows the County to claim its allocation of MSSP funding for FY 2008-09 in the amount of $771,300, and FY 2009-10 in the amount of $771,300. The total amount of this Agreement is $1,542,600. Revenues and expenses related to this Agreement have been included in the AAS tentatively Adopted FY 2008-09 and FY 2009-10 Budgets. There is no Net County Cost associated with approving this Agreement.
