RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AGREEMENT NUMBER MS-0809-13 WITH THE CALIFORNIA DEPARTMENT OF AGING, FOR FUNDING IN THE AMOUNT OF $1,542,600 FOR THE CONTINUATION OF THE MULTIPURPOSE SENIOR SERVICES PROGRAM FOR THE TERM OF JULY 1, 2008 THROUGH JUNE 30, 2010; AND 2) AUTHORIZING THE DIRECTOR OF HEALTH OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE FUNDING AMOUNT BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby the Aging and Adult Services Division shall provide a Multipurpose Senior Services Program (MSSP) that provides comprehensive social and health support necessary to enable frail elderly to remain in their own homes; and

WHEREAS, both parties now wish to enter into an Agreement to provide MSSP services for the period of July 1, 2008 through June 30, 2009 in the amount of $771,300 and for the period of July 1, 2009 through June 30, 2010 in the amount of $771,300 for a total obligation of $1,542,600; and

WHEREAS, this Board has examined the Agreement and has approved it as to both form and content:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Agreement with the California Department of Aging for the MSSP funds for the term of July 1, 2008 through June 30, 2010 for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of the Health Department or the Director’s designee be and is hereby authorized to execute contract amendments which modify the funding amount by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
