RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH THE CITY OF DALY CITY ADDING FAMILY HEALTH COMMUNITY WORKER SERVICES FOR THE PERIOD JULY 1, 2008 THROUGH JUNE 30, 2009, AND INCREASING THE MAXIMUM OBLIGATION BY $67,296 FROM $310,380 TO $377,676

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on April 8, 2008 this Board approved an Agreement with the City of Daly City to provide mental health counselor and nutrition education services for the term January 1, 2008 through June 30, 2010 for a maximum obligation of $310,380; and

WHEREAS, both parties now wish to amend the Agreement to add family health community worker services and to increase the maximum obligation by $67,296 for a new maximum obligation of $377,676; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby the City of Daly City shall provide family health community worker services for County of San Mateo; and

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
