RESOLUTION AUTHORIZING A) EXECUTION OF AN AGREEMENT WITH YOUTH AND FAMILY ENRICHMENT SERVICES (YFES) TO PROVIDE DIFFERENTIAL RESPONSE CASE MANAGEMENT SERVICES FOR THE SOUTHERN REGION FOR THE TERM JULY 1, 2008 TO JUNE 30, 2011, IN THE AMOUNT OF $1,787,460, THERE IS NO NET COUNTY COST; AND B) THE DIRECTOR OF THE HUMAN SERVICES AGENCY OR THE DIRECTOR’S DESIGNEE TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPE OF SERVICES AND ACTIVITIES TO BE PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, in February, 2008, a Request for Proposal (RFP) was conducted for the provision of Differential Response case management services in the North, Central and South regions of San Mateo County; and

WHEREAS, the County of San Mateo wishes to enter into a three-year Agreement with YFES pursuant to which YFES will provide Differential Response services to at-risk families in the Southern Region, for which San Mateo County’s maximum payment is $1,787,460 for the term of July 1, 2008 through June 30, 2011; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that:

A) The President of this Board of Supervisors is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto; and

B) The Director of the Human Services Agency or the Director’s designee is authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000 and to make minor changes in the type of services and activities provided under said Agreement.
