RESOLUTION AUTHORIZING AN AMENDMENT TO THE AGREEMENT WITH SAMARITAN HOUSE (HUMBOLDT) TO PROVIDE FUNDING FOR THE REMAINING TERM OF THE CORE SERVICE AGREEMENT THE TERM REMAINS THE SAME, JULY 1, 2007 THROUGH JUNE 30, 2010, INCREASING THE AMOUNT BY $231,482 TO $351,570, $216,482 IS NET COUNTY COST

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 5, 2007, the Board of Supervisors adopted Resolution Number 068764, waiving the Request for Proposal (RFP) process, authorizing Core Services in the amount of $120,088, for FY 20007-08 only, and authorized the Director of the Human Services Agency to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000, and to make minor changes in the types of services and activities provided under the Agreement; and

WHEREAS, the term of the Agreement will remain the same, July 1, 2007 through June 30, 2010, this Amendment will provide $231,482 for the remaining term of the Agreement, for a new total obligation of $351,570; and

WHEREAS, this Board has been presented with the form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into said Amendment.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
