RESOLUTION AUTHORIZING (A) A SECOND AMENDMENT TO THE AGREEMENT WITH SECOND HARVEST FOOD BANK TO PROVIDE FUNDING FOR THE REMAINING TERM OF THE AGREEMENT THE TERM REMAINS THE SAME,

JULY 1, 2007 THROUGH JUNE 30, 2010, INCREASING THE AMOUNT BY $130,380 TO $230,380; $130,380 IS NET COUNTY COST; AND (B) THE DIRECTOR OF THE HUMAN SERVICES AGENCY OR THE DIRECTOR’S DESIGNEE TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 14, 2007, the County Manager’s Office approved Agreement Number 71000-08-C030 authorizing an Agreement with Second Harvest, for the purpose of providing the Operation Brown Bag and the Food Assistance Program; and

WHEREAS, on January 30, 2008, the County Manager’s office approved Change Order Number 1 to include the Food Stamp Outreach Program; and

WHEREAS, the parties now wish to further amend the Agreement to provide funding for the Operation Brown Bag and the Food Assistance Programs in the amount of $130,380 for a total obligation of $230,380; and

WHEREAS, this Board has been presented with the form of this Second Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that

A. The President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

B. The Director of the Human Services Agency or the Director’s designee shall be authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000, and to make minor changes in the types of services and activities provided under the Agreement.
