RECOMMENDATION:

Adopt a Resolution:
|

1.
|
Authorizing the execution of an Amendment to the Agreement with Retina-Vitreous Associates, Inc. to provide professional Ophthalmologic Surgical Services, increasing the maximum amount payable thereunder by $65,000 from $65,000 to $130,000.
|

2.
|
Authorizing the Chief Executive Officer of SMMC or the CEO’s designee to execute contract Amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term of services is/are within the current or revised fiscal provisions.

BACKGROUND:
|
On July 7, 2007, the County Manager approved an Agreement with Retina-Vitreous Associates, Inc. to provide ophthalmologic surgical services to patients referred by the San Mateo Medical Center. The term of the agreement is April 1, 2007 to March 31, 2009 in an amount not to exceed $65,000.

DISCUSSION:
|
Historically, agreements with Retina-Vitreous Associates, Inc. cover a one year term. However, the current Agreement was made for a two year period and the amount allocated is only sufficient to fund services for a one year period.
|
|
Consequently, the Agreement needs to be amended to increase funding to allow for payment of additional services provided during the contract period. This amended will increase the funding by $65,000, for a total not to exceed $130,000.
|
|
This item was reviewed by the Health System Redesign Initiative Team and aligns with the first-year implementation plan approved by the Board of Supervisors on March 25, 2008.
|

County Counsel and Risk Management have reviewed and approved this Agreement.
|
|
The contractor has assured compliance with the County’s Contractor Employee Jury Service Ordinance, as well as all other contract provisions that are required by the County ordinance and administrative memoranda, including but not limited to insurance, hold harmless, non-discrimination and equal benefits.
|

FISCAL IMPACT:
|
The term of the Agreement is April 1, 2007 through March 31, 2009. The maximum amount of the Agreement, as proposed to be amended is $130,000. Funds are included in San Mateo Medical Center’s FY 2007-08 Adopted Budget and will be included in the budget request for FY 2008-09.
