BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

ADOPT A RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A THIRD AMENDMENT TO THE AGREEMENT WITH THE STATE OF CALIFORNIA MANAGED RISK MEDICAL INSURANCE BOARD TO: ESTABLISH NEW ADMINISTRATIVE POLICIES IN EXHIBITS A AND B OF THE AGREEMENT; EXTEND THE TERM OF THE AGREEMENT FROM JULY 1, 2005 THROUGH JUNE 30, 2008 TO JULY 1, 2005 THROUGH JUNE 30, 2009; AND INCREASE THE FUNDING AMOUNT BY $354,586, FOR A NEW MAXIMUM OF $1,733,225

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, in December 2005 the Board of Supervisors adopted Resolution 67753, approving an Agreement with the State of California’s Managed Risk Medical Insurance Board to receive federal matching funds for the Children’s Health Initiative Program; and

WHEREAS, in February 2008 the Board of Supervisors adopted Resolution 69285 extending the term of the Agreement to July 1, 2005 through June 30, 2008 and approving a new rate not to exceed $75.67 per-member per-month for Healthy Kids children ages one through eighteen years; and

WHEREAS, both parties now wish to establish new administrative policies in Exhibits A and B of the Agreement, extend the term of the Agreement to July 1, 2005 through June 30, 2009, and increase the funding amount by $354,586 for a new maximum of $1,733,225; and

WHEREAS, this Board of Supervisors has been presented with the Amendment to the Agreement and has examined and approved this Amendment as to form and content and desires to amend the Agreement:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute the Amendment for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
