RESOLUTION AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE CAREADVANTAGE AGREEMENT WITH SAN MATEO HEALTH COMMISSION FOR THE PROVISION OF MEDICARE SERVICES FOR HEALTH PLAN OF SAN MATEO CLIENTS FOR THE TERM JANUARY 1, 2008 TO DECEMBER 31, 2008, INCREASING THE INPATIENT PER DIEM RATE BY $275.57 FROM $760 TO $1,035.57 EFFECTIVE JULY 1, 2008

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the CareAdvantage Agreement with San Mateo Health Commission for the provision of Medicare Services for Health Plan of San Mateo (HPSM) clients for the term January 1, 2008 to December 31, 2008; and

WHEREAS, the CareAdvantage Agreement provides for the care of inpatient, outpatient and emergency services patients who are eligible to receive CareAdvantage Benefits; and

WHEREAS, HPSM reimburses SMMC for providing such care; and

WHEREAS, the parties now wish to amend the Agreement to change the current inpatient per diem rate of $760/per diem to the new rate of $1,035.57/per diem, effective July 1, 2008; and

WHEREAS, this Board has been presented with a form of the Amendment Agreements and has examined and approved it as to both form and content and desires to enter into it:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
