AMENDMENT TO AGREEMENT
BETWEEN THE COUNTY OF SAN MATEO AND
ASSOCIATED MEDICAL SPECIALISTS

THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and ASSOCIATED MEDICAL SPECIALISTS, hereinafter called "Contractor";

|
W I T N E S S E T H:
|
|
|
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
|
|
|
WHEREAS, the parties entered into an Agreement for internal medicine services on November 8, 2005; and
|
|
|
WHEREAS, the parties wish to amend the Agreement to increase the maximum amount payable under the Agreement by $894,571.58, from $11,080,524.59 to $11,975,096.17.

|
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
|

The last paragraph of Schedule B of the original agreement is amended to read as follows:
The total payment for services under this Agreement will not exceed ELEVEN MILLION, NINE HUNDRED SEVENTY FIVE THOUSAND, NINETY SIX DOLLARS AND SEVENTEEN CENTS ($11,975,096.17).

All other terms and conditions of the agreement dated November 8, 2005, between the County and Contractor shall remain in full force and effect.
