RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT TWO TO THE AGREEMENT WITH CATHOLIC CHARITIES CYO FOR THE CONTINUATION OF THE MEALS ON WHEELS AND SUPPLEMENTAL MEALS ON WHEELS PROGRAMS INCREASING THE AMOUNT BY $111,232 TO $698,593 AND EXTENDING THE TERM THROUGH SEPTEMBER 30, 2008

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
WHEREAS, this Board has designated itself as the Area Agency on Aging (AAA) of San Mateo County to carry out programs pursuant to the Older Americans Act (OAA); and
|
|
WHEREAS, it is necessary and desirable that the AAA administer the programs of the OAA, Community-Based Services Programs (CBSP), and Health Insurance Counseling and Advocacy Program (HICAP); and
|
|
WHEREAS, on August 14, 2007, by Resolution 68920, the Board authorized execution of 23 Agreements for an aggregate total of $3,317,549 for the term July 1, 2007 through June 30, 2008 to provide an array of services to at-risk individuals in San Mateo County; and
|
|
WHEREAS, on April 8, 2008, by Resolution 69343, the Board authorized Amendment One to 16 of the Agreements increasing the OAA, CBSP, and HICAP funding amount by $247,564 thus changing the total amount under the Agreements from $3,317,549 to $3,565,113 with no change to the term of the Agreements; and

WHEREAS, Amendment One further amended the Agreement with Ombudsman Services of San Mateo County, Inc., by incorporating State required language regarding a Grievance Procedure specific to that Agreement; and

|
WHEREAS, on June 17, 2008, the Board authorized Resolution 69500 allowing the Controller’s Office to continue payments on the original 23 Agreements through September 30, 2008 with no change to the amounts or terms for FY 2007-08; and
|
|
WHEREAS, on March 20, 2008 Catholic Charities CYO submitted a 30-day notice informing Aging and Adult Services (AAS) that it would no longer be providing the Meals on Wheels (MOW) and Supplemental Meals on Wheels (SMOW) Programs in San Mateo County, but would continue to provide the service until another provider was located or September 30, 2008, whichever came first; and

WHEREAS, AAS conducted a Request for Proposals for the MOW and SMOW Programs for the term July 1, 2008 through June 30, 2009 and only one proposal was submitted and accepted to cover a portion of the catchment area served by Catholic Charities CYO; and
|
|
WHEREAS, the parties wish to further amend the Agreement with Catholic Charities CYO by increasing the amount by $111,232 for a total of $698,593 and extending the term from July 1, 2007 through September 30, 2008 for the continuation of the MOW and SMOW Programs; and
|
|
WHEREAS, all other services provided through the Agreement with Catholic Charities CYO will be continued in accordance to the continuing resolution; and
|
|
WHEREAS, the Board has been presented with Amendment Two to the Agreement with Catholic Charities CYO and desires to enter into same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
