ADOPT A RESOLUTION AUTHORIZING THE FIRST AND SECOND AMENDMENTS TO THE AGREEMENT WITH THE DEPARTMENT OF REHABILITATION FOR TRAINING AND EMPLOYMENT SERVICES FOR MENTAL HEALTH CONSUMERS, THE TERM REMAINS JULY 1, 2007 THROUGH JUNE 30, 2010, THE AMOUNT ALLOCATED TO THE COUNTY BY THE STATE HAS INCREASED BY $84,850 TO $2,630,350, NO NET COUNTY COST

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on August 14, 2007, the Board of Supervisors adopted Resolution number 068925 authorizing the execution of a revenue Agreement with the State of California, Department of Rehabilitation for the purpose of providing funding for situational assessment, work adjustment, employment preparation, job development and placement services for County mental health consumers; and

WHEREAS, the original Agreement with the Department of Rehabilitation allocated the amount of $2,545,500 to the County over a three year term of July 1, 2007 through June 30, 2010; and

WHEREAS, the County and the Department of Rehabilitation wish to amend the Agreement for the purpose of adjusting the cash match amounts through a First Amendment and adding an additional allocation from the Department of Rehabilitation to the County for continued services through a Second Amendment in the amount of $84,850 ($42,425 for FY 2008-09 and $42,425 for FY 2009-10) for a total allocation of $2,630,350 and the term remaining July 1, 2007 through June 30, 2010; and

WHEREAS, this Board has been presented with a form of such First and Second Amendments to the Agreement and said Board has examined and approved same as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said First and Second Amendments to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
