RESOLUTION A) AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH LABVANTAGE FOR THE PROVISION OF OVERSIGHT AND SUPERVISION FOR THE POINT OF CARE TESTING (POCT) PROGRAM AT SAN MATEO MEDICAL CENTER EXTENDING THE TERM BY SIX MONTHS TO DECEMBER 31, 2008, AND INCREASING THE AMOUNT BY $64,749.92 TO THE MAXIMUM AMOUNT NOT TO EXCEED $129,499.92 B) AUTHORIZING THE CHIEF EXECUTIVE OFFICER (CEO) OF SAN MATEO MEDICAL CENTER (SMMC) OR THE CEO'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of

California, that

WHEREAS, the San Mateo Medical Center has a current need for oversight and supervision for the Point of Care Testing (POCT) program; and

WHEREAS, LabVantage is willing and able to provide oversight and supervision for POCT; and

WHEREAS, the County of San Mateo and LabVantage entered into an Agreement dated January 22, 2008, whereby LabVantage agrees to provide oversight and supervision for the POCT program; and

WHEREAS, the parties now wish to amend the Agreement for the provision of POCT oversight and supervision to extend the term of the Agreement through December 31, 2008 and to increase the total amount expendable thereunder by $62,749.92 to $129,499.92; and

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signatures thereto. .

BE IT FURTHER DETERMINED AND ORDERED that the Chief Executive Officer of SMMC shall be authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
