RESOLUTION AUTHORIZING THE EXECUTION OF A THIRD AMENDMENT TO THE AGREEMENT WITH PER DIEM STAFFING SYSTEMS, INC. TO PROVIDE SAN MATEO MEDICAL CENTER (SMMC) WITH TEMPORARY RESPIRATORY THERAPY STAFFING, TO INCREASE THE MAXIMUM AMOUNT PAYABLE BY $7,500 TO $347,500

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS; the County and Per Diem Staffing Systems, Inc. entered into an Agreement for the purpose of providing San Mateo Medical Center (SMMC) with temporary respiratory staffing on April 4, 2006, by Resolution 67908; and subsequently amended on June 19, 2007, by Resolution 68808 and on February 26, 2008 by Resolution 069300; and

WHEREAS, SMMC continues to require the use of traveler staff in order to maintain minimum Respiratory Department staffing levels; and

WHEREAS, Per Diem Staffing is willing to provide part-time temporary respiratory services to SMMC; and

WHEREAS, the parties wish to further amend their Agreement to increase maximum amount payable thereunder by $7,500 from $340,000 to $347,750; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and approval, a Third Amendment to the Agreement between the County of San Mateo and Per Diem Staffing Systems, Inc., reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same.

NOW THEREFORE, IT IS HEREBY RESOLVED that the Board of Supervisors be, and is hereby authorized and directed to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
