RESOLUTION AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH JULIETTE LEE, M.D., TO PROVIDE PROFESSIONAL DERMATOLOGY SERVICES AT SAN MATEO MEDICAL CENTER FOR THE TERM SEPTEMBER 1, 2008 THROUGH AUGUST 31, 2009 IN AN AMOUNT PAYABLE NOT TO EXCEED $150,000 AND 2) AUTHORIZING THE CHIEF EXECUTIVE OFFICER (CEO) OF SAN MATEO MEDICAL CENTER (SMMC) OR THE CEO’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the County has an ongoing need for Dermatology services at San Mateo Medical Center which Juliette Lee, M.D. is willing to provide for up to two full days per week; and

 

WHEREAS, there has been presented to the Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby Dr. Juliette Lee shall provide the County with professional Dermatology services for the period September 1, 2008 through August 31, 2009 for a total amount payable not to exceed $150,000; and

 

WHEREAS, the Board has considered the Agreement and approved it as to both form and content and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signatures thereto.

 

BE IT FURTHER RESOLVED that the Chief Executive Officer of San Mateo Medical Center shall be authorized to execute Amendments and minor modifications not to exceed $25,000 and to make minor changes in the types of services and activities provided under the Agreement.
