BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION 1) AUTHORIZING THE EXECUTION OF AN AGREEMEMNT WITH FRED LUI, MD, TO PROVIDE NEPHROLOGY SERVICES FOR THE TERM OCTOBER 1, 2008 THROUGH SEPTEMBER 30, 2011, IN AN AMOUNT NOT TO EXCEED $450,000; AND 2) AUTHORIZING THE CHIEF EXECUTIVE OFFICER (CEO) OF SAN MATEO MEDICAL CENTER (SMMC) OR THE CEO’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the County of San Mateo has a continuous need for nephrology services; and

 

WHEREAS, Fred Lui, M.D. offers the County the benefits of an experienced practitioner familiar with the complex nature of the County’s patient base; and

 

WHEREAS, there has been presented to the Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Fred Lui, M.D. shall provide the County of San Mateo with nephrology services for the period October 1, 2008 through September 30, 2011, for total compensation not to exceed $450,000; and

 
 

WHEREAS, this Board has considered the form of the Agreement and has approved it as to both form and content and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief Executive Officer of San Mateo Medical Center or the CEO’s designee shall be authorized to execute contract Amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term of services is/are within the current or revised fiscal provisions
