BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH EAR, NOSE, AND THROAT GROUP OF SAN MATEO TO PROVIDE OTORHINOLARYNGOLOGY MEDICAL SERVICES FOR THE TERM OCTOBER 1, 2008 THROUGH SEPTEMBER 30, 2010, IN AN AMOUNT NOT TO EXCEED $612,000 AND 2) AUTHORIZING THE CHIEF EXECUTIVE OFFICER (CEO) OF SAN MATEO MEDICAL CENTER OR THE CEO’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, San Mateo Medical Center has a continued need for otorhinolaryngology (ENT) medical services; and

WHEREAS, Ear, Nose, and Throat Group of San Mateo is willing to continue to provide these necessary services; and

WHEREAS, both parties now wish to enter into an Agreement whereby Ear, Nose, and Throat Group of San Mateo will provide ENT medical services for the period October 1, 2008 through September 30, 2010 for a total amount not to exceed $612,000; and

WHEREAS there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Ear, Nose, and Throat Group of San Mateo shall continue to provide ENT medical services; and

WHEREAS, this Board has examined and approved this agreement as to both form and content and desires to enter into it:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED that the Chief Executive Officer of San Mateo Medical Center shall be authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
