BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE ACCEPTANCE OF A GRANT FROM SILICON VALLEY COMMUNITY FOUNDATION IN THE AMOUNT OF $2,000 TO PROVIDE AN UPGRADE TO THE DATABASE SOFTWARE FOR PATIENT TRACKING

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, Peninsula Community Foundation created the original database software for the Adult Dental Program and has funded upgrades to the software each year; and

WHEREAS, Peninsula Community Foundation and Community Foundation Silicon Valley merged to become the Silicon Valley Community Foundation; and

WHEREAS, enhancement of the original database software will improve patient tracking; and

WHEREAS, SMMC has received a grant award of $2,000 from Silicon Valley Community Foundation to provide the support necessary for the Information Services Department at San Mateo Medical Center to upgrade the database software; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a grant award in the amount of $2,000 which is hereby made for further particulars, whereby the Silicon Valley Community Foundation will allocate funds for the provision of an upgrade to the database software for patient tracking; and

WHEREAS, this Board has considered the grant award and desires to accept it.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to accept said award for and on behalf of the County of San Mateo, and to execute any documentation necessary to effectuate such acceptance, and the Clerk of this Board shall attest the President’s signatures thereto.
