RECOMMENDATION:

Adopt a resolution:

 

A.

Authorizing the President of the Board to execute an Agreement with Richard Hom, O.D. to provide primary care optometry services for the term September 1, 2008 through August 31, 2010 in an amount not to exceed $237,573.50

   

B.

Authorizing the Chief Executive Officer of San Mateo Medical Center or designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions

BACKGROUND:

Dr. Richard Hom has provided optometry services, which include general eye health, the diagnosis and identification for diseases of the eye, and clinics for eye injury, trauma, or refractive corrections, for many years. Patients treated at SMMC have a continuous need for such eye care services. Dr. Hom offers the hospital the benefit of an experienced provider familiar with the complex nature of the County’s patient base. In addition, Dr. Hom will provide the service of retinal reading and the interpretation of retinal photos for two-three hours per week.

DISCUSSION:

To meet the ongoing demand for optometry services, Dr. Hom will continue to provide ambulatory primary care optometry clinic services on a full time basis in clinics located at South San Francisco, Ron Robinson Senior Care Center, and the main campus of SMMC.

Signed agreement was returned by Dr. Hom on September 30, 2008 which caused the delay in a more timely submission of this Agreement to the Board.

This item was reviewed and approved by the Health System Redesign Initiative Team, County Counsel and Risk Management.

The contractor has assured compliance with the County's Contractor Employee Jury Service Ordinance, as well as all other Agreement provisions that are required by County ordinance and administrative memoranda, including but not limited to insurance, hold harmless, non-discrimination and equal benefits.

TERM AND FISCAL IMPACT:

The term of the Agreement is from September 1, 2008 through August 31, 2010 for a total not to exceed $237,573.50. The yearly cost of this Agreement is $118,786.75 per year for two years. There is no increase in the annual rate of the previous agreement. Funds are included in SMMC's FY2008-09 Adopted Budget and will be requested in the FY2009-10 Budget.
