RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO AGREEMENT EPO CDC 07-41 WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH TO PROVIDE PUBLIC HEALTH EMERGENCY PREPAREDNESS FOR THE PERIOD OF AUGUST 31, 2007 THROUGH AUGUST 9, 2008, INCREASING THE AGREEMENT MAXIMUM BY $931,906 TO A NEW MAXIMUM OF $1,114,173

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on May 6, 2008, your Board authorized Agreement EPO CDC 07-41 for health emergency preparedness providing $182,267 for the term August 31, 2007 through August 9, 2008; and

WHEREAS, both parties now wish to amend the Agreement to increase the total funding by $931,906 for a new maximum of $1,114,173 for the term August 31, 2007 through August 9, 2008; and

WHEREAS, this Board has been presented with this amended Agreement and has examined and approved it as to both form and content and desires to enter into the amended Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
