RESOLUTION 1)WAIVING THE REQUEST FOR PROPOSALS (RFP)PROCESS, 2) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH THE CITY OF DALY CITY TO PROVIDE CUSTODIAL AND MAINTENANCE SERVICES FOR THE DALY CITY CLINIC/ MIKE NEVIN HEALTH CENTER FOR THE TERM JUNE 1, 2008 TO MAY 31, 2010, IN AN AMOUNT NOT TO EXCEED $585,000, AND 3) AUTHORIZING THE CHIEF EXECUTIVE OFFICER OF SAN MATEO MEDICAL CENTER OR THE CEO’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OF SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, Ordinance Code Section 2.83.170 authorizes the Board of Supervisors to waive the Request for Proposals process in any situation where the Board of Supervisors determines that the best interest of the county could be served without the necessity of requests for proposals; and

 

WHEREAS, the parties wish to enter into a new Agreement with City of Daly City pursuant to which the City will provide custodial and maintenance services to the Daly City Clinic/Mike Nevin Health Center, for the period June 1, 2008 to May 31, 2010, for a total obligation not to exceed $585,000; and

 

WHEREAS, the Board has considered the Agreement and approved it as to both form and content and desires to enter into it.

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Request for Proposal process is hereby waived; and

IT IS HEREBY FURTHER RESOLVED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED that the Chief Executive Officer of San Mateo Medical Center or the CEO’s designee shall be authorized to execute contract Amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term of services is/are within the current or revised fiscal provisions.
