RESOLUTION 1) AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH FOR THE IMMUNIZATION ASSISTANCE PROGRAM FOR THE TERM OF JULY 1, 2008 THROUGH JUNE 30, 2009 IN THE AMOUNT OF $252,918; AND 2) AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH FOR THE IMMUNIZATION REGISTRY FOR THE TERM OF JULY 1, 2008 THROUGH JUNE 30, 2009 IN THE AMOUNT OF $35,164

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance two Agreements, reference to which is hereby made for further particulars, whereby the California Department of Public Health (CDPH) shall provide funding to San Mateo County for immunization assistance and immunization registry activities in the total amount of $288,082 for the term of July 1, 2008 through June 30, 2009; and

WHEREAS, this Board has examined and approved them as to both form and content and desires to enter into said Agreements:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
