RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT ONE TO THE AGREEMENT WITH DANIEL T. KRESTELLER, C.P.A., M.S., FOR CERTIFIED PUBLIC ACCOUNTANT/TAX PREPARER SERVICES FOR AGING AND ADULT SERVICES’ CONSERVED CLIENTS INCREASING THE TOTAL AMOUNT BY $15,000 FROM $90,000 TO $105,000 FOR THE TERM DECEMBER 1, 2007 THROUGH DECEMBER 31, 2008 WITH THE OPTION TO RENEW FOR TWO YEARS; AND 2) AUTHORIZING THE DIRECTOR OF HEALTH OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on November 28, 2007, the County Manager approved Agreement 57000-08-C162 with Mr. Kresteller for the provision of Certified Public Accountant/Tax Preparer services in the amount of $90,000 for the term December 1, 2007 through December 31, 2008 with the option to renew for two additional years; and

WHEREAS, both parties now wish to amend the Agreement with Mr. Kresteller for the reimbursement of unanticipated services provided due to the one-time Federal stimulus claims filed in 2007 for Aging and Adult Services’ Public Guardian clients for the period of December 1, 2007 through December 31, 2008, increasing the total obligation by $15,000 from $90,000 to $105,000; and

WHEREAS, the Board has been presented with a form of such Amendment One to the Agreement and said Board has examined and approved same as to both form and content and desires to enter into said Amendment:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board be and is hereby authorized and directed to execute said Amendment with Daniel T. Kresteller, C.P.A., M.S., for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of Health or the Director’s designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
