RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH ONE EAST PALO ALTO TO ADD MULTI-CULTURAL CENTER SERVICES AND TO INCREASE FUNDING FOR OUTREACH SERVICES, INCREASING THE MAXIMUM OBLIGATION BY $182,585 TO A NEW MAXIMUM OF $321,395 WITH NO CHANGE TO THE TERM OF JULY 1, 2008 THROUGH JUNE 30, 2009

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 24, 2008, this Board approved an Agreement whereby One East Palo Alto provides technical consultation for the East Palo Alto Mental Health Community Advisory Group and mental health outreach and engagement services for a maximum obligation of $138,810 for the term July 1, 2008 through June 30, 2009; and

 

WHEREAS, both parties now wish to amend the Agreement to add multi-cultural center services and add funding for additional outreach and engagement services; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby One East Palo Alto shall provide multi-cultural center services, effective November 1, 2008, and additional outreach and engagement services, effective January 1, 2009, increasing the maximum obligation by $182,585 to a new maximum of $321,395 with no change to the Agreement term; and

 

WHEREAS, this Board has been presented with the amended Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

 

NOW THEREFORE IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
