RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH DANIEL BUCKLEY, M.D. FOR OPHTHALMOLOGY SERVICES FOR THE TERM SEPTEMBER 1, 2008 THROUGH AUGUST 31, 2011, IN AN AMOUNT NOT TO EXCEED $254,700 AND 2) AUTHORIZING THE CHIEF EXECUTIVE OFFICER OF SAN MATEO MEDICAL CENTER OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000, AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, San Mateo Medical Center (SMMC) has engaged in extensive negotiations for ophthalmology services for the past year which resulted in a collective contract with three providers, one of which is addressed by this Resolution; and

WHEREAS, SMMC wishes to enter into an Agreement with Daniel Buckley, M.D. to provide ophthalmology services for the period September 1, 2008 through August 31, 2011, for a maximum amount not to exceed $254,700; and

WHEREAS there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Daniel Buckley, M.D. shall provide professional ophthalmology services at an increased level of service and productivity; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED that the Chief Executive Officer of SMMC or designee shall be authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000, and/or modify the contract’s term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
