RESOLUTION 1) CORRECTING THE AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH FOR THE IMMUNIZATION ASSISTANCE PROGRAM FOR THE TERM OF JULY 1, 2008 THROUGH JUNE 30, 2009 IN THE AMOUNT OF $252,918; AND 2) CORRECTING THE AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH FOR THE IMMUNIZATION REGISTRY FOR THE TERM OF JULY 1, 2008 THROUGH JUNE 30, 2009 IN THE AMOUNT OF $35,164

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on November 4, 2008, the Board of Supervisors approved the execution of two Agreements, (Resolution No.069781) whereby the California Department of Public Health (CDPH) shall provide funding to San Mateo County for immunization assistance and immunization registry activities in the total amount of $288,082 for the term of July 1, 2008 through June 30, 2009; and

WHEREAS, the first page of the Immunization Assistance Program Agreement erroneously omitted the following language: “The Information Systems Security Requirements for Projects (Exhibit G) is not applicable for this contract. No confidential data for this contract is exchanged between CDPH and the LHD.”; and

WHEREAS, the first page of the Immunization Registry Agreement erroneously omitted the following language: “The SRI: Information Systems Security Requirements for Projects (Exhibit H) is not applicable for this contract. No confidential data for this contract is exchanged between CDPH and the LHD.”:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that these Agreements shall be corrected by replacing the first page of these Agreements to include the above language and the President of the Board of Supervisors be and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, pursuant to Resolution No. 069781, and the Clerk of this Board shall attest the President’s signature thereto.
