RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH FRED FINCH YOUTH CENTER FOR MENTAL HEALTH SERVICES FOR CHILDREN AND YOUTH INCREASING THE MAXIMUM OBLIGATION BY $129,715 TO A NEW MAXIMUM OF $1,231,689, WITH NO CHANGE TO THE TERM OF JULY 1, 2007 THROUGH JUNE 30, 2008

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on September 25, 2007, this Board: 1) approved an Agreement with Fred Finch Youth Center for the provision of Crisis In-Home Stabilization services, Therapeutic Behavioral Services, Day Treatment Services and Visiting Therapist outpatient mental health services for the term July 1, 2007 through June 30, 2008, for a maximum obligation of $1,081,974; and 2) authorized the Director of Health or the Director's designee to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions; and

 

WHEREAS, on August 14, 2008, the Director of Health approved an Amendment to the Agreement increasing the Agreement maximum by $20,000 for additional visiting therapist services to a new maximum of $1,101,974 with no change to the Agreement term; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Second Amendment to the Agreement, reference to which is hereby made for further particulars, to increase the maximum obligation by $129,715 to a new maximum $1,231,689 with no change to the Agreement term; and

 

WHEREAS, this Board has examined the Second Amendment to the Agreement and has approved it as to both form and content and desires to enter into the Second Amendment to the Agreement:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
