RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH CURELINE BIOPATHOLOGY, LLC TO PROVIDE ANATOMIC PATHOLOGY SLIDE PREPARATION AND HISTOLOGY SERVICES FOR SURGICAL AND BIOPSY PROCEDURES PERFORMED AT SAN MATEO MEDICAL CENTER EXTENDING THE TERM BY BY FIVE MONTHS TO DECEMBER 31, 2008 AND INCREASING THE MAXIMUM AMOUNT BY $75,000 TO $195,500.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, Cureline BioPathology, LLC (Cureline) was the only responder to a Request for Proposals (RFP) for laboratory services; and

 

WHEREAS, the Blue Ribbon Task Force is negotiating with several hospitals and providers to provide various services, including the type provided by Cureline; and

 

WHEREAS, SMMC wishes to have uninterrupted laboratory services until the Blue Ribbon Task Force has completed its negotiations and entered into agreements for services; and

 

WHEREAS, the parties wish to amend the Agreement with Cureline to extend the term of November 6, 2007 to December 31, 2008; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to an Agreement, reference to which is hereby made for further particulars, whereby Cureline shall provide anatomic pathology slide preparation and histology services for surgical and biopsy procedures performed at San Mateo Medical Center; and

 

WHEREAS, the Board has considered the Amendment and approved it as to both form and content and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
