AMENDMENT TO AGREEMENT
BETWEEN THE COUNTY OF SAN MATEO AND
THE REGENTS OF THE UNIVERSITY OF CALIFORNIA on behalf of UCSF MEDICAL CENTER, UCSF CHILDREN’S HOSPITAL, AND UCSF MEDICAL GROUP

THIS AMENDMENT TO THE AGREEMENT, entered into this _______ day of __________, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and THE REGENTS OF THE UNIVERSITY OF CALIFORNIA on behalf of UCSF MEDICAL CENTER, UCSF CHILDREN’S HOSPITAL, AND UCSF MEDICAL GROUP, a California constitutional Corporation, hereinafter called "Contractor";

W I T N E S S E T H:
|
|
|
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof; and
|
|
|
WHEREAS, the parties entered into an Agreement for diagnostic and tertiary care services on August 15, 2006; and
|
|
|
WHEREAS, the parties wish to amend the Agreement to increase the maximum amount payable thereunder by $953,000, from $450,000 to $1,403,000.
|
|
|
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
|

Paragraph 2 of Section 3, Payments, of the Agreement is amended to read as follows:
Payments The County reserves the right to withhold payment if the County determines that the quantity or quality of the work performed is unacceptable. In no event shall the County’s total fiscal obligation under this Agreement exceed $1,403,000.

The last paragraph of original Exhibit B is amended to read as follows:
The term of this Agreement is January 1, 2006 to December 31, 2008. In no event shall total payment payable under this Agreement exceed $1,403,000.

|
|
3.
|
All other terms and conditions of the Agreement dated August 15, 2006 between the County and Contractor shall remain in full force and effect.
|
|
|
|
|
