BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH BREATHE CALIFORNIA: GOLDEN GATE PUBLIC HEALTH PARTNERSHIP THAT SUPERSEDES THE INTERIM AGREEMENT SIGNED ON AUGUST 29, 2008, TO PROVIDE TOBACCO CESSATION SERVICES AND A PUBLIC EDUCATION CAMPAIGN ON THE IMPACT OF TOBACCO USE IN FILMS FOR THE TERM OF JULY 1, 2008 THROUGH SEPTEMBER 30, 2009, FOR A MAXIMUM OBLIGATION OF $132,000; AND 2) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, this Agreement supersedes the interim Agreement 62000-09-C106 signed by the County Manager on August 29, 2008; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement with Breathe California to provide tobacco cessation services, and to execute a public campaign to educate the public regarding the harm to youth of exposure to tobacco use in film media, for the term of July 1, 2008 through September 30, 2009, for a total maximum obligation of $132,000; and

 

WHEREAS, this Board has been presented with a form of such Agreement and said Board has examined and approved same as to both form and content and desires to enter into same:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is authorized to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Director of the Health Department or designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
