RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A GRANT AGREEMENT WITH SILICON VALLEY COMMUNITY FOUNDATION IN THE AMOUNT OF $230,364 FOR CONTINUED SUPPORT OF THE HEALTHIER OUTCOMES THROUGH MULTIDISCIPLINARY ENGAGEMENTS TEAM FOR THE TERM SEPTEMBER 1, 2008 THROUGH AUGUST 31, 2010; AND 2) AUTHORIZING THE HEALTH DIRECTOR OR DESIGNEE TO EXECUTE ALL NECESSARY APPLICATIONS, CONTRACTS, AGREEMENTS, AMENDMENTS AND PAYMENT REQUESTS HERETO FOR THE PURPOSES OF IMPLEMENTING THE GRANT AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on October 17, 2006, your Board approved the establishment of the Aging and Adult Services, the Healthier Outcomes Through Multi-disciplinary Engagement (HOME) Team; and

 

WHEREAS, your Board also adopted Resolution 68334 authorizing a grant agreement with the Silicon Valley Community Foundation, formerly known as the Peninsula Community Foundation, in the amount of $100,000 for the term December 1, 2006 through November 20, 2007; and

 

WHEREAS, by Resolution 68973, your Board authorized Amendment One to the grant agreement extending the term from December 1, 2006 through November 30, 2007 to December 1, 2006 through May 31, 2008, with no change to the amount of $100,000; and

 

WHEREAS, the HOME Team has been dedicated to securing resources for immediate needs of clients, such as connecting patients who frequently rely on the Emergency Department for routine healthcare to appropriate primary care, housing and services for substance abuse; and

 

WHEREAS, both parties now wish to enter into a Grant Agreement, whereby the Silicon Valley Community Foundation will provide continued funding to support the HOME Team for the period September 1, 2008 through August 31, 2010, for a total obligation of $230,364; and

 

WHEREAS, this Board has been presented with a form of such Grant Agreement and said Board has examined and approved same as to both form and content and desires to enter into said Agreement:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Grant Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

IT IS FURTHER DETERMINED AND ORDERED that the Health Director or designee is hereby authorized to accept or execute on behalf of the County, any necessary applications, contracts, agreements, amendments and payment requests hereto for the purposes of implementing the Grant Agreement.
