BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION (1) WAIVING THE REQUEST FOR PROPOSAL PROCESS; (2) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT NO.2 TO THE AGREEMENT WITH MAXONIC MILESTONE MANAGEMENT FOR PROFESSIONAL SERVICES, INCREASING THE AMOUNT BY UP TO $63,000, FOR A NEW MAXIMUM TOTAL FISCAL OBLIGATION OF $138,000; AND (3) AUTHORIZING THE CHIEF INFORMATION OFFICER OR HIS DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on October 12, 2008, the Information Services Department entered into an Agreement in the amount of $25,000 for the term of October 12, 2008, to June 30, 2009, with Maxonic Milestone Management to provide the Information Services Department with professional services relating to the County’s Internet and Intranet;

WHEREAS, on December 4, 2008, the County Manager approved a Change Order to the Agreement to increase the amount by up to $50,000;

WHEREAS, this Board having considered the matter has determined that it would be in the best interest of the County to waive the Request for Proposal process and approve an Amendment to the Agreement with Maxonic Milestone Management to increase the contract amount by up to $63,000, for a new total fiscal obligation not to exceed $138,000;

WHEREAS, this Board has been requested to grant signature authority to the Chief Information Officer or his designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the Contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions; and

WHEREAS, this Board has been presented with a form of the Amendment, has examined and approved it as to both form and content, and desires to enter into same;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Request for Proposal process is hereby waived, the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for an on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that signature authority is granted to the Chief Information Officer or his designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
