Acceptance of an Award from the California Department of Public Health Tuberculosis Control Branch for the Northern California Regional Civil Detention at Cordilleras Mental Health Rehabilitation Center

 

RECOMMENDATION

Adopt a Resolution authorizing the President of the Board to execute an Acceptance of Award in the amount of $187,600 from the California Department of Public Health, Tuberculosis Control Branch for the Northern California Regional Civil Detention at Cordilleras Mental Health Rehabilitation Center that supersedes the Acceptance of Award in the amount of $87,600 that was signed on November 14, 2008, for the term of July 1, 2008 through June 30, 2009.

The Tuberculosis (TB) grant award contributes to this commitment and goal by providing funds to detain persistently non-adherent TB patients and appropriately treat them to prevent spread of the disease.

BACKGROUND

Tuberculosis (TB) is a highly contagious disease that can be treated and contained with the correct medications. However, some patients fail to comply with medication orders. Each county has a Directly Observed Therapy program which provides for staffing to monitor patients in the field to ensure they are appropriately taking their medications. Unfortunately, some patients miss appointments, are difficult to find, and do not comply with their physician’s orders. A county health officer can order a tuberculosis patient admitted to a facility to ensure that the patient receives medication and the community remains safe.

 

DISCUSSION

Telecare Corporation operates Cordilleras Center, the Regional Civil Detention Site for non-adherent TB patients in Northern California. There are only two sites available in the State for this purpose. Due to the existing relationship between the Health System and Telecare, and the location of Cordilleras Center within the County, the State has requested that the County contract with Telecare for this service. One bed is dedicated at the Cordilleras Center for use by health jurisdictions in the region that need to detain persistently non-adherent TB patients. However, since October 2008 up to three non-adherent TB patients at a time have been admitted to the facility, each from a different jurisdiction, requiring an increase in the Acceptance of Award from the State.

 

The TB Program operates in compliance with the requirements of the California Department of Public Health (CDPH) Policies and Procedures Manual for FY 2008-09 as well as conditions stipulated by the State TB Control Branch.

 

The work required by the terms of the Agreement has been undertaken by the Health Department on written assurance and direction of CDPH.

 

The Acceptance of Award and Resolution have been reviewed and approved by County Counsel.

 

FISCAL IMPACT

The term of the Acceptance of Award remains July 1, 2008 through June 30, 2009. The total amount of the Award is $187,600, an increase of $100,000. The revenue and expenditures associated with this Award are included in the Community Health FY 2008-09 Adopted Budget. There are no required matching funds included in this award, resulting in no Net County Cost.
