BACKGROUND: |
Since 2004 the One-e-App (OeA) system has been used by Certified Application Assistants in San Mateo County to screen and help families apply for health coverage programs through the Children’s Health Initiative (CHI). In August 2007 your Board approved an Agreement with The Center to Promote HealthCare Access (The Center), to maintain OeA as the system used to screen all uninsured adults and children for San Mateo County’s health coverage program. These programs include: Medi-Cal; Healthy Kids; Healthy Families; the San Mateo Access and Care for Everyone (ACE); Discounted Healthcare Program; and other publicly-funded health coverage programs, such as the Family Planning, Access, Care, and Treatment, Cancer Detection Program, and the Child Health, Disability, and Prevention programs. In February 2008 your Board approved an Amendment with The Center to modify the OeA system logic to include new ACE policies, eligibility requirements and determination processes. |
In December 2008 as one of the final planning phase recommendations of the Blue Ribbon Task Force on Adult Health Care Coverage Expansion, your Board approved the transition of the administration of the County’s local adult indigent care program, formerly known as the Wellness, Education, Low-Cost and Linkage (WELL) program from the San Mateo Medical Center to the Health Plan of San Mateo (HSPM). |
DISCUSSION: |
Effective January 1, 2009, all of the County’s adult indigent health care programs administered by the HPSM are known as the ACE program. This program consolidation includes the WELL program and the ACE program and provides a more seamless and streamlined health coverage application process for clients. |
Through this proposed second Amendment, The Center will provide system enhancements and maintenance of OeA to support this consolidation. The Center will modify the OeA’s system logic to include the new ACE and Medi-Cal policies, and local eligibility determination processes and requirements. Because of the tight turnaround time between the adoption of relevant policies approved by your Board in December and the Amendment beginning date needed to enable seamless contractor support, this Amendment is late and will not be executed as of its start date of January 27, 2009. |

FISCAL IMPACT: |
The first Amendment increased the Agreement amount by $153,412, for a maximum obligation of $781,576, with no change to the Agreement term, July 1, 2007 through June 30, 2010. This second Amendment increases the Agreement amount by $212,484 from $781,576 for a new maximum obligation of $994,060. The total Agreement costs are shared by CHI ($406,331), Human Services Agency ($254,934), Health Plan of San Mateo ($9,873), and the San Mateo Medical Center ($322,922). The FY 2008-09 Adopted Budget for CHI is $1.7 million, of which $266,043 is allocated to this Agreement. The remaining costs for this fiscal year have been included in FY 2008-09 Adopted Budgets of the Human Services Agency and San Mateo Medical Center. Costs for subsequent years will be included in their respective budgets. The Net County Cost for this Amendment is $50,548. |
