RESOLUTION (1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT NO. 1 TO THE AGREEMENT WITH SYSAPPROACH, INC. FOR PROFESSIONAL SERVICES, INCREASING THE AMOUNT BY UP TO $150,000, FOR A NEW MAXIMUM TOTAL FISCAL OBLIGATION OF $238,400, AND EXTENDING THE TERM OF THE AGREEMENT TO DECEMBER 31, 2009; AND (2) AUTHORIZING THE CHIEF INFORMATION OFFICER OR HIS DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 3, 2008, the County entered into an Agreement in the amount of $88,400 for the term of September 3, 2008, to March 6, 2009, with SysApproach, Inc. to provide the Information Services Department with professional services;

WHEREAS, this Board having considered the matter has determined that it would be in the best interest of the County to approve an Amendment to the Agreement with SysApproach, Inc. to increase the contract amount by up to $150,000, for a new total fiscal obligation not to exceed $238,400, and to extend the term of the Agreement to December 31, 2009;

WHEREAS, this Board has been requested to grant signature authority to the Chief Information Officer or his designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the Contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions; and

WHEREAS, this Board has been presented with a form of the Amendment, has examined and approved it as to both form and content, and desires to enter into same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is authorized and directed to execute said Amendment to the Agreement as is approved by the County Counsel for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that signature authority is granted to the Chief Information Officer or his designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
