AMENDMENT TO AGREEMENT
BETWEEN THE COUNTY OF SAN MATEO AND
THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and HEALTH ADVOCATES, LLC hereinafter called "Contractor";
W I T N E S S E T H:
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
WHEREAS, on June 6, 2006, the parties entered into an Agreement (for the County by Resolution 068043) for cost recovery and legal services (the “Agreement”);
WHEREAS, the parties wish to amend the Agreement to increase the maximum amount payable thereunder by $1,000,000, from $2,280,000 to $3,280,000.
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
Paragraph 3, Payments, of the Agreement is hereby amended in its entirety to read as follows:
Section F of the original Exhibit B is amended in its entirety to read as follows:
The term of this Agreement is May 1, 2006, to April 30, 2009. Total payments for services provided under this Agreement will not exceed THREE MILLION TWO HUNDRED EIGHTY THOUSAND DOLLARS ($3,280,000). Invoices will be approved by the Patient Access Manager and paid within 30 days of receipt of invoice.
