RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH UNITY CARE GROUP, INC. FOR MENTAL HEALTH SERVICES, INDIVIDUAL AND GROUP THERAPY, MEDICATION SUPPORT SERVICES, CRISIS INTERVENTION SERVICES AND THERAPEUTIC BEHAVIORAL SERVICES, FOR THE TERM AUGUST 28, 2008 THROUGH JUNE 30, 2010, FOR A MAXIMUM OBLIGATION OF $280,900; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR THE CHIEF’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

 
 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, both parties now wish to enter into an Agreement whereby the Unity Care Group, Inc. shall provide mental health services, individual and group therapy, medication support services, crisis intervention services and Therapeutic Behavioral Services for the term August 28, 2008 through June 30, 2010, for a maximum obligation of $280,900; and

 

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement.

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is authorized to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or the Chief’s

 

designee be and is hereby authorized to execute contract amendments which modify the

 

County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify

 

the contract term and/or services so long as the modified term or services is/are within the

 

current or revised fiscal provisions.
