RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT ONE TO THE AGREEMENT WITH TELECARE CORPORATION FOR DIRECT OBSERVATION THERAPY SERVICES FOR NON-ADHERENT TUBERCULOSIS PATIENTS AT CORDILLERAS MENTAL HEALTH CENTER TO INCREASE THE PAYMENT RATE FROM $236 PER BED/DAY TO $240 PER BED/DAY, AND TO INCREASE THE MAXIMUM BY $101,460 FROM $7,297,050 TO A NEW MAXIMUM OF $7,398,510 FOR THE TERM OF JULY 1, 2008 THROUGH JUNE 30, 2009

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the parties entered into an Agreement for inpatient and residential psychiatric treatment services, and direct observation therapy services for non-adherent tuberculosis patients at Cordilleras Mental Health Center for the term of July 1, 2008 through June 30, 2009, for a maximum obligation of $7,297,050; and

WHEREAS, the parties now wish to amend the Agreement to increase the payment rate from $236 per bed/day to $240 per bed/day, and to increase the maximum obligation by $101,460 for a new maximum obligation of $7,398,510; and

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to execute the Amendment.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute Amendment One to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
