BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO EXECUTE A CONTRACT WITH CHASE CONTRACTING, INC., DBA QUALITY AIR SERVICES IN THE AMOUNT OF $68,820.00 FOR THE BURLINGAME LONG TERM CARE FACILITY – CLEANING OF HVAC DUCTWORK, AND 2) AUTHORIZING THE DIRECTOR OF PUBLIC WORKS TO EXECUTE CONTRACT REVISION ORDERS RELATED TO THE FACILITY’S CLEANING OF HVAC DUCTWORK NOT EXCEEDING $10,323.00 IN AGGREGATE, WHICH IS 15% OF THE CONTRACT AMOUNT.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Board of Supervisors did, on January 27, 2009, adopt Resolution No. 069905, which Resolution adopted and approved the plans and specifications, determined prevailing wage scales and called for sealed bids for the Burlingame Long Term Care Facility – Cleaning of HVAC Ductwork, and

WHEREAS, the clerk of this Board of Supervisors did, in public on February 24, 2009 open and examine all sealed proposals, and

WHEREAS, the Board of Supervisors has caused an analysis of said sealed proposals to be made by the Director of Public Works of the County of San Mateo, and has, in open session, fully reviewed and considered said proposals and the analysis thereof; and

WHEREAS, Chase Contracting, Inc. dba Quality Air Services submitted the lowest responsible bid, in the amount of Sixty Eight Thousand Eight Hundred and Twenty and No/100 dollars ($68,820.00) based on the estimate of the amount of the work to be done.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED as follows:

It is in the best interest of the County of San Mateo to award the contract for the Burlingame Long Term Care Facility – Cleaning of HVAC Ductwork to:

Chase Contracting, Inc. dba Quality Air Services

Hayward, CA 94544

2.

The County of San Mateo shall enter into a written contract with Chase Contracting, Inc. dba Quality Air Services for the doing of said work as required by the plans and specifications adopted and approved by this Board of Supervisors, as hereinbefore mentioned.

The President of the Board of Supervisors shall be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

4.

All payments to the Contractor shall be in accordance with the plans, specifications, and other contract documents. Aggregate payment to Contractor shall not exceed the sum of Sixty Eight Thousand Eight Hundred and Twenty and No/100 dollars ($68,820.00) as may be amended by authorized revision orders without this Board’s approval.

5.

The Director of Public Works shall be and is hereby authorized to execute contract revision orders related to the Burlingame Long Term Care Facility – Cleaning of HVAC Ductwork not exceeding $10,323 in aggregate, which is 15% of the contract amount.
