RESOLUTION AUTHORIZING THE EXECUTION OF AMENDMENT TWO TO AGREEMENT HI-0809-08 WITH THE CALIFORNIA DEPARTMENT OF AGING FOR THE ADDITION OF $24,099, IN ONE-TIME-ONLY FUNDING, INCREASING THE TOTAL FROM $268,853 TO $292,952 FOR FY 2008-09

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
|
WHEREAS, this Board has designated itself as the Area Agency on Aging of San Mateo County to carry out programs pursuant to the Older Americans Act of 1965; and
|
|
WHEREAS, on June 3, 2008, your Board of Supervisors, by Resolution 69458, authorized Agreement HI-0809-08 with the California Department of Aging (CDA) for Health Insurance Counseling and Advocacy Program (HICAP) funding in the amount of $246,053 for the term July 1, 2008 through June 30, 2009; and

WHEREAS, Resolution 69458 authorized the Director of Health or the Director’s designee to execute limited contract amendments; and
|
|
WHEREAS, on January 14, 2009, the Chief of the Health System approved the first amendment to Agreement HI-0809-08, increasing the amount by $22,800 to $268,853 with no change to the term; and
|
|
WHEREAS, both parties now wish to further amend the Agreement by increasing the amount by $24,099 changing the maximum obligation from $268,853 to $292,952 for the purpose of providing Low-Income Subsidy outreach and enhancing services for older adults in the community with no change to the term; and
|
|
WHEREAS, the Board of Supervisors has been presented with this Amendment and desires to enter into same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to Agreement HI-0809-08 with CDA for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
