RESOLUTION AUTHORIZING THE PUBLIC WORKS DIRECTOR TO EXECUTE ADDITIONAL REVISION ORDERS TO THE CONTRACT WITH STOLOSKI AND GONZALES, INC. IN AN AMOUNT NOT TO EXCEED $23,500, FOR A TOTAL REVISION ORDER AUTHORITY NOT TO EXCEED $58,500, AND AN AGGREGATECONTRACT AMOUNT OF NOT TO EXCEED $415,000 (MIDCOAST DRAINAGE IMPROVEMENT PROJECT – CYPRESS AVENUE FROM HIGHWAY 1 TO ETHELDORE STREET, ETHELDORE STREET FROM SUNSHINE VALLEY ROAD TO VERMONT AVENUE, AND SUNSHINE VALLEY ROAD FROM STETSON STREET TO ETHELDORE STREET – MOSS BEACH AREA (PROJECTS NO. M0210, M0211, F-149 [21])

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on November 18, 2008 the Board of Supervisors adopted Resolution No. 069804, which increased the contract with Stoloski and Gonzales, Inc. by $35,000 for a total contract amount of $391,500 in order to include the cost for work to relocate existing Montara Water and Sanitary District water mains that were in conflict with the proposed storm drainage improvements; and

 

WHEREAS, on July 22, 2008 the Board of Supervisors adopted Resolution No. 069601, approving revised plans and specifications, determining prevailing wage scales and authorizing execution of a contract with Stoloski & Gonzales, Inc., hereinafter referred to as “Contractor,” for the Midcoast Drainage Improvement Project; and

 

WHEREAS, additional unforeseen utility conflicts were encountered during construction and design changes were made in order to avoid said conflicts; and

 

WHEREAS, to avoid conflicts with sewer and water mains, the proposed storm drain line along Sunshine Valley Road and Etheldore Street was re-aligned, requiring two additional concrete junction structures to be installed and additional asphalt concrete pavement at the intersection of these two streets; and

 

WHEREAS, a drainage headwall and slope protection were installed at the south end of the new box culvert in Cypress Avenue for erosion control purposes and to help ensure continuous drainage to the existing ditch located in State right-of-way; and

 

WHEREAS, an increase in the Contract amount of $23,500 is required to cover the cost for the additional work necessitated by design changes to the storm drainage improvements; and

 

WHEREAS, the Board has considered and concurs with the recommendation of the Director of Public Works.

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that:

 

In addition to the previously authorized contract revision orders in the amount of $35,000 that the Board of Supervisors authorized the Director of Public Works to make for the agreement with Stoloski and Gonzales, Inc. for the MidCoast Drainage Improvement Project – Cypress Avenue from Highway 1 to Etheldore Street, Etheldore Street from Sunshine Valley Road from Stetson Street to Etheldore Street – Moss Beach Area, the Director of Public Works shall be and is hereby authorized to execute contract revision orders related to the Storm Drainage Improvements in an additional amount not to exceed $23,500, for a total revision order authority not to exceed $58,500, and an aggregate contract amount not to exceed $415,000.
