RESOLUTION A) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH ON-SITE DENTAL CARE FOUNDATION TO PROVIDE DENTAL HEALTH CARE SERVICES TO HOMELESS INDIVIDUALS IN SAN MATEO COUNTY FOR THE TERM NOVEMBER 1, 2008 THROUGH OCTOBER 31, 2009, IN AN AMOUNT NOT TO EXCEED $156,840; AND B) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, San Mateo Medical Center (SMMC) was awarded the Health Care for the Homeless (HCH) grant from the Federal Bureau of Primary Health Care of the Health and Resource Services Administration to provide primary, dental and behavioral health care to San Mateo County’s homeless population; and

WHEREAS, HCH is designed to assist homeless adults, families and youth with access to and utilization of the SMMC’s continuum of primary care, dental care, mental health, substance abuse and other support services; and

WHEREAS, prior to issuing a Request for Proposals (RFP) in 2007, the HCH Program, led by its Advisory Committee, implemented a planning process which included completing a strengths, weaknesses, opportunities and threats analysis, revising the Program’s mission and creating a description of vision and values; and

WHEREAS, the HCH Program chose to fund two County programs and three community-based programs, one of which is On-Site Dental Care Foundation, to provide services to homeless individuals in San Mateo County; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby On-Site Dental Care Foundation shall provide dental health care service for up to 384 homeless individuals for the period November 1, 2008, through October 31, 2009, for a total fiscal obligation not to exceed $156,840; and

WHEREAS, the Board has considered the Agreement and approved it as to both form and content and desires to enter into it.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or

designee shall be authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions of the Agreement.
