RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH SHELTER NETWORK TO PROVIDE BEHAVIORAL HEALTH CARE SERVICES TO SAN MATEO COUNTY’S HOMELESS POPULATION FOR THE TERM OF NOVEMBER 1, 2008 TO OCTOBER 31, 2009, IN AN AMOUNT NOT TO EXCEED $221,075 AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, San Mateo Medical Center (SMMC) was awarded the Health Care for the Homeless (HCH) grant from the Federal Bureau of Primary Health Care of the Health and Resource Services Administration to provide primary, dental, and behavioral health care to San Mateo County’s homeless population; and

 

WHEREAS, Shelter Network has been selected by the County as a community-based program to receive HCH funding to provide behavioral health care services to individuals who are homeless and/or at risk of being homeless; and

 

WHEREAS, the County and Shelter Network wish to enter into an Agreement whereunder Shelter Network will provide behavioral health care services to San Mateo County’s homeless population for the term of November 1, 2008, through October 31, 2009, in the amount of no more than $221,075; and

 

WHEREAS, the Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signatures thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
