RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT A02 TO AGREEMENT NO. 07-65080 WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH FOR THE TERM OF JULY 1, 2007 THROUGH JUNE 30, 2010, REDUCING THE CONTRACT AMOUNT BY $170,122, DECREASING THE MAXIMUM OBLIGATION FROM $5,119,734 TO $4,949,612.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Amendment A02 to an Agreement with the California Department of Public Health, reference to which is hereby made for further particulars, whereby the County shall provide HIV prevention, HIV counseling and testing, AIDS case management, HIV/AIDS surveillance, and an early intervention program; and

WHEREAS, the term of the Agreement is July 1, 2007 through June 30, 2010, and the contract amount for years two and three has been reduced by $170,122, decreasing the maximum obligation from $5,119,734 to $4,949,612; and

WHEREAS, this Board has been presented with the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
