RESOLUTION AUTHORIZING (A) A SECOND AMENDMENT TO THE AGREEMENT WITH THE KATE BRISTOL CONSULTING (KBC) TO PROVIDE CONSULTING SERVICES FOR THE SAN MATEO COUNTY HOMELESS CONTINUUM OF CARE (COC) APPLICATION, HOPE (HOUSING OUR PEOPLE EFFECTIVELY) IMPLEMENTATION, AND COMPLETING THE JANUARY 2009 REQUIRED HOMELESS CENSUS AND SURVEY REPORTS, THE TERM REMAINS THE SAME, AUGUST 15, 2007 THROUGH AUGUST 14, 2010; INCREASING THE AMOUNT BY $69,250 TO $168,750; $69,250 IS NET COUNTY COST; AND (B) THE DIRECTOR OF THE HUMAN SERVICES AGENCY OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on August 9, 2007, the County Manager’s Office approved an Agreement with Kate Bristol Consulting , Agreement Number 71000-08-C080, waiving the Request for Proposal (RFP) process, authorizing services in the amount of $50,000, for FY 2007-08 only; and

 

WHEREAS, on September 18, 2008, the County Manager’s Office approved a First Amendment to the Agreement increasing the amount by $49,500 for a total of Agreement amount of $99,500; and

 

WHEREAS, the term of the Agreement remains the same, August 15, 2007 through August 14, 2010; and

 

WHEREAS, the Second Amendment adds the technical support services for the Inter Agency Council (IAC) and Lead Person Collaborative (LPC) and increases the total obligation by $99,500 to $168,750; and

 

WHEREAS, the Second Amendment authorizes the Director of the Human Services Agency to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000, and to make minor changes in the types of services and activities provided under the Agreement; and

 

WHEREAS, this Board has been presented with the form of the Second Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into said Amendment; and

 

    NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that

A. The President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Second Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

B. The Director of the Human Services Agency or the Director’s designee shall be authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000, and to make minor changes in the types of services and activities provided under the Agreement.
