RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AGREEMENT NO. AP-0910-08 FOR FUNDING UNDER TITLES III AND VII OF THE OLDER AMERICANS ACT AND FOR THE COMMUNITY-BASED SERVICES PROGRAM OF THE OLDER CALIFORNIANS ACT WITH THE CALIFORNIA DEPARTMENT OF AGING FOR THE CONTINUATION OF NUTRITION, SUPPORTIVE, AND EMPLOYMENT COMMUNITY-BASED SERVICES FOR THE TERM OF JULY 1, 2009 THROUGH JUNE 30, 2010 IN THE AMOUNT OF $2,905,278; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE FUNDING AMOUNT BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, this Board has designated itself as the Area Agency on Aging of San Mateo County to carry out programs supporting senior services; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby the Aging and Adult Services Division shall provide Adult Day Care, Alzheimer’s Program, Brown Bag, Case Management, Congregate Nutrition, Disease Prevention/Health Promotion, Family Caregiver Support, Home-Delivered Meals, Legal Assistance, Ombudsman Program, Transportation Program, and Unsubsidized Senior Employment services; and

 
 

WHEREAS, both parties now wish to enter into an Agreement to provide Titles III and VII funding of the Older Americans Act (OAA) and Community-Based Services Programs (CBSP) funding of the Older Californians Act (OCA) programs for the period of July 1, 2009 through June 30, 2010 for a total obligation of $2,905,278; and

 

WHEREAS, this Board has examined the Agreement and has approved it as to both form and content.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Agreement with the California Department of Aging for the OAA Titles III and VII and OCA CBSP funds for the term July 1, 2009 through June 30, 2010 for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee be authorized to execute contract amendments which modify the funding amount by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
