BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION : 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH CHARIS CHILDREN’S CENTER FOR DAY TREATMENT INTENSIVE SERVICES, MEDICATION SUPPORT SERVICES, MENTAL HEALTH SERVICES, CRISIS INTERVENTION SERVICES AND THERAPEUTIC BEHAVIORAL SERVICES FOR SAN MATEO COUNTY YOUTH, INCREASING THE AGREEMENT MAXIMUM BY $304,819 TO A NEW MAXIMUM OF $394,819 AND EXTENDING THE TERM THROUGH JUNE 30, 2010; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 13, 2008, the County Manager approved an Agreement with Charis Children’s Center for the period July 1, 2008 through June 30, 2009 for Day Treatment Intensive Services, medication support services, mental health services, crisis intervention services, and Therapeutic Behavior Services for San Mateo County youth for a maximum obligation of $90,000; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to that Agreement, reference to which is hereby made for further particulars, whereby Charis Children’s Center shall provide Day Treatment Intensive Services, medication support services, mental health services, crisis intervention services and Therapeutic Behavior Services for additional San Mateo County youth; and

WHEREAS, both parties now wish to amend the Agreement to extend the term through June 30, 2010, and increase the maximum obligation by $304,819 to a new maximum obligation of $394,819; and

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is hereby authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
