BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: (1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AGREEMENTS WITH CATHOLIC CHARITIES CYO; THE CITY OF SOUTH SAN FRANCISCO ADULT DAY CARE PROGRAM; AND PENINSULA VOLUNTEERS, INC., FOR ADULT DAY CARE SERVICES FOR THE TERM JULY 1, 2009 THROUGH JUNE 30, 2013, IN THE COLLECTIVE AMOUNT OF $141,600; AND (2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the parties wish to enter into the Agreements to provide adult day care services to Aging and Adult Services’ referred clients for the term July 1, 2009 through June 30, 2013, in the collective amount of $141,600; and

 

WHEREAS, amounts to be paid for these services in subsequent fiscal years will be determined at a later date; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, Agreements between the County of San Mateo and Catholic Charities CYO; the City of South San Francisco Adult Day Care Program; and Peninsula Volunteers, Inc.; and

 

WHEREAS, the Board of Supervisors has been presented with these Agreements and desires to enter into same.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee shall be authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
