BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH INFOIMAGE, INC. FOR PROCESSING, PRINTING AND MAILING SERVICES OF PATIENT BILLS AND SERVICE STATEMENTS/INVOICES FOR THE TERM JULY 1, 2009 THROUGH JUNE 30, 2014, FOR AN AMOUNT NOT TO EXCEED $989,280; AND 2) AUTHORIZING THE CHIEF OF HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, Environmental Health (EH), Behavioral Health and Recovery Services (BHRS), and San Mateo Medical Center (SMMC) require the services of a vendor to process, print, and mail patient bills and services statements; and

WHEREAS, in January 2009 EH, BHRS, and SMMC conducted a joint Request for Proposals (RFP) for printing and mail processing of patient bills and service statements/invoices; and

WHEREAS, InfoImage, Inc. met all the requirements of the RFP and was awarded the agreement based on program evaluation, cost, appropriate resources to provide the service, and experience in the industry; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement whereby InfoImage, Inc. shall process, print, and mail patient bills and service statements/invoices for Behavioral Health and Recovery Service, Environmental Health, and San Mateo Medical Center; and

WHEREAS, both parties now wish to enter into an Agreement for processing, printing, and mailing of patient bills and service statements/invoices for the period of July 1, 2009 to June 30, 2014, for a total obligation of $989,280; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute this Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of Health System or designee shall be authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
