RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT TWO TO EACH OF THE AGREEMENTS WITH ADDUS HEALTHCARE, INC.; AMERICAN CAREQUEST, INC.; MEDICAL CARE PROFESSIONALS; AND NP PLUS, LLC FOR HOME HEALTH AND ATTENDANT CARE SERVICES AMENDING SERVICE RATES AND INCREASING THE COLLECTIVE AMOUNT BY $800,000 FOR FY 2009-10 FOR A TOTAL OF $2,570,000 WITH NO CHANGE TO THE TERM OF JULY 1, 2007 THROUGH JUNE 30, 2010

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on August 14, 2007, this Board adopted Resolution 68921 approving Agreements with Addus HealthCare, Inc.; American CareQuest, Inc.; Medical Care Professionals; and NP Plus, LLC for the provision of home health and attendant care services in the amount of $970,000 for the term July 1, 2007 through June 30, 2010; and

WHEREAS, on June 17, 2008, this Board adopted Resolution 69497 approving the First Amendments to the Agreements adding language regarding the maximum allowable charge for reimbursement when a client refuses or is not available for services to be no greater than one hour and increasing the collective amount by $800,000 for the period July 1, 2008 through June 30, 2009, changing the total obligation to $1,770,000 with no change to the term July 1, 2007 through June 30, 2010; and

WHEREAS, the parties wish to further amend the Agreements by amending service rates and increasing the collective amount by $800,000 for the period July 1, 2009 through June 30, 2010, changing the total obligation to $2,570,000 with no change to the terms July 1, 2007 through June 30, 2010, for the purpose of continuing home health and attendant care services; and

WHEREAS, this Board has examined the Second Amendments to the Agreements and has approved them as to both form and content.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Second Amendments to the Agreements for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
